Terms & Conditions

1. Original and physical voucher must be presented during appointment day.
2. Valid for redemption at HMI Medical Centre located at 12 Farrer Park Station Road, #01-01, Singapore 217565, strictly by appointment only.
3. Any additional services, tests or upgrades beyond the Classic screening package will be at the individual’s expense.
4. Valid till 30 June 2025. No extension of the validity period will be granted.
5. Non-refundable or exchangeable for cash.
6. Voucher cannot be combined with other promotions and discounts.
7. Voucher cannot be replaced or extended if lost, damaged or expired.
8. HMI Medical Centre reserves the right to vary or amend the privileges or terms & conditions without prior notice.
9. HMI Medical Centre shall not be responsible for any claim, demand, loss, damage, expense or liability of any kind whatsoever, arising from or in connection with any unauthorised usage of this voucher.
