Programme rates are available to all Singlife customers, whether for a first consultation or a follow-up review or refill. Your final fee will depend on the medication and dosage prescribed by your attending doctor. Additional consultations, assessments, tests, delivery or follow-up care may incur separate charges.

    Your doctor will discuss an appropriate care plan and treatment options with you.

  4. Begin your care journey

    If medically suitable, begin prescribed treatment. Follow-up consultations can be completed in-clinic or via teleconsultation.

Who is eligible for the Singlife Chronic Care Programme?

Programme rates are available to all Singlife customers. At your appointment, just show proof that you’re a Singlife customer, such as your Singlife app or policy details, so the clinic can verify your eligibility for the programme rates.

Medical suitability for treatment is determined separately by the attending doctor following an assessment.

Do I need a referral to book?

No referral is required. You can book an appointment directly with any participating clinic, or via teleconsultation.

Is this service available if I’m not a Singlife customer?

Yes. The service is available to non-Singlife customers, but Singlife programme rates will not apply. Standard consultation rates and applicable charges will apply.

Are programme rates the same across participating clinics?

Yes. Participating HMI clinics follow the same Singlife programme rate structure for all Singlife customers. The published rates are from rates for consultations and prescribed medication, so your final cost may vary depending on the medication prescribed, dosage and treatment required. Any additional tests, services or treatment will be charged separately.

What happens at my first consultation?

Your first consultation must be an in-person visit at a participating HMI clinic, as it includes a medical assessment.

Your doctor will review your health, medical history, current medications, weight-related health risks and goals before discussing an appropriate care plan with you.

Depending on your individual needs, additional assessments or tests may be recommended. Where relevant, your doctor may also advise further care or referral for other health concerns identified during the assessment.

How will my treatment be decided?

Your care plan will be based on your medical assessment, health needs, goals and relevant clinical guidelines.

Treatment may include lifestyle changes and, where medically appropriate, prescription medication such as GLP-1 therapy. Your doctor will determine the appropriate treatment, medication type and dosage for you.

Your care plan may also be adjusted over time based on your progress, response to treatment and any changes in your health.

Can follow-up consultations be done by teleconsultation?

Your first consultation must be completed in person at a participating HMI clinic, as it involves a medical assessment.

Once your care plan has been established, follow-up consultations can be completed in-clinic or via teleconsultation, where medically appropriate. Follow-up reviews allow your doctor to assess your progress, discuss what is working and adjust your care plan where needed.

I’m already on prescribed weight management medication. How can I get a review or refill?

You can book a follow-up review either in clinic or via teleconsultation, depending on your preference and your doctor’s assessment.

If you started treatment with another clinic or doctor, you may be asked to attend an in-person consultation first. Please bring or have on hand any recent prescriptions, test results or relevant medical records.

Any prescription or refill is subject to your doctor’s clinical assessment.

Can I refill my prescribed weight management medication without seeing a doctor?

No. Prescription medication requires a medical review before a refill can be provided. Depending on your treatment and your doctor’s assessment, your follow-up review may be done in clinic or via teleconsultation.

Are there any delivery fees for medication after a teleconsultation?

For Singlife customers, the medication delivery fee is waived under the Singlife Chronic Care Programme.

Simply show or mention your Singlife app (logged in) or policy number during your teleconsultation so the team can verify your Singlife customer status.

What if medication is not suitable for me, or I decide not to proceed?

Medication is only one part of weight management and will only be prescribed where medically appropriate.

Your doctor can discuss other aspects of your care, including lifestyle changes, follow-up and any other health needs identified during your assessment.

You will only be charged for the consultation and any additional services, tests or treatment you receive. You will not be charged for medication that is not prescribed or dispensed.

How will my progress be assessed?

Progress is different for each person and is not based on weight alone.

Your doctor may consider changes in your weight alongside your overall health, treatment response and other relevant health markers, depending on your individual needs and goals.

Regular follow-up helps your doctor assess your progress and adjust your care plan where appropriate.

How long does treatment last?

There is no fixed duration, as weight management is different for each person.

Your doctor will recommend a care and follow-up plan based on your health needs, goals and progress over time. Regular reviews help assess what is working, monitor relevant health markers and adjust your care plan where needed.

The aim is to support sustainable progress rather than a short-term result.

What does the “From $289/month” rate include?

The rate includes a GP consultation and prescribed medication, where medically suitable.

Your final cost will depend on the medication and dosage prescribed by your attending doctor.

Additional consultations, assessments, tests, delivery or follow-up care may incur separate charges.

Can I use CHAS subsidies or MediSave for my weight management treatment?

CHAS subsidies and MediSave may be used for eligible components of your care if you have a qualifying chronic condition, such as diabetes, high blood pressure or high cholesterol.

Eligibility depends on your medical condition, CHAS or MediSave eligibility, the clinic you visit and prevailing claim limits. Not all consultations, tests or medications may be claimable.

Is this claimable under my Singlife policy?

No. The published rates are programme rates available to Singlife customers and are not claimable or reimbursable under your Singlife policy. You’ll pay the applicable fees directly to the clinic.

Who delivers the programme?

The Singlife Chronic Care Programme is a joint partnership between Singlife, Novo Nordisk and HMI Medical.

Care is provided through HMI Managed Healthcare and participating HMI clinics, including HMI Managed Healthcare Clinic and selected HMI OneCare clinics.

Treatment suitability and prescribing decisions are made by the attending doctor following a medical assessment. Singlife does not determine individual treatment decisions.

Are there additional Singlife benefits if I make progress through the programme?

Participants who meet the relevant criteria at their post-programme assessment may be eligible for preferential rates on selected Singlife insurance plans.

This includes participants who achieve a BMI below 30 kg/m², subject to applicable programme terms, underwriting requirements and eligibility criteria.
